Your health and wellness are the main interests of the physicians and staff at East Rockingham Health Center. We invite you to participate as an equal partner in your health care.

Appointments
Except for walk-in patients seen during our walk-in hours, we see patients on an appointment basis. When scheduling an appointment with our office, please inform us of any recent hospital or nursing home stays, or emergency room visits.
We make every effort to honor all time commitments and ask that you extend the same courtesy to us. On occasion, emergencies may cause a delay in our schedule. As far as it is possible, we will keep you informed of such delays. If you are unable to keep your appointment, please call as early as possible in advance in order to avoid a missed appointment charge. Excessive appointment absences may result in dismissal from the practice.
Your Visit
| 1. |
Please bring all current medications with you. |
| 2. |
Check in with the receptionist at each visit, present your insurance card and notify us of any changes in your address or telephone number. |
| 3. |
If you are self-pay or uninsured, payment in full is expected at the time of your visit, unless prior arrangements have been made. We do offer a 20 percent discount for office visit charges that are paid in full at the time of the visit. |
| 4. |
We bill all types of insurances; however, you are responsible for your insurance co-pay.You will be expected to pay this at the time of your visit. You are also responsible for non-covered fees and deductibles. We accept major credit cards, checks and cash. Depending upon your insurance coverage, you may be responsible for charges related to X-ray or lab services. In addition, your insurance may require you to call ahead for determination of coverage for your visit and/or certain medications. |
| 5. |
Your insurance policy may require that we provide you with a referral form in order for you to visit physicians outside our practice, including ER physicians. Make sure to secure a referral from us before your appointment with an outside physician. Failure to do so may result in additional medical expenses for you. |
| 6. |
If your insurance does not list one of our providers as your primary care provider (PCP), you will need a referral prior to your appointment here. |
