Appointments
We see patients on an appointment basis. When scheduling an appointment with our office, please inform us of any recent hospital or nursing home stays, or emergency room visits.
We make every effort to honor all time commitments and ask that you extend the same courtesy to us. On occasion, emergencies may cause a delay in our schedule. As far as it is possible, we will keep you informed of such delays. If you are unable to keep your appointment, please call as early as possible in advance in order to avoid a missed appointment charge. Excessive appointment absences may result in dismissal from the practice.
Your Visit
- Please bring all current medications with you.
- Check in with the receptionist at each visit, present your insurance card and notify us of any changes in your address or telephone number.
- If you are self-pay or uninsured, payment in full is expected at the time of your visit, unless prior arrangements have been made. We do offer a 20 percent discount for office visit charges and procedures to our self-pay patients.
- We bill all types of insurances; however, you are responsible for your insurance co-pay. You will be expected to pay this at the time of your visit. You are also responsible for non-covered fees and deductibles. We accept major credit cards, checks and cash.
- If your insurance does not list one of our providers as your primary care provider (PCP), you will need to contact your insurance company to ensure we are a participating provider.
- Please avoid using your cell phone in the clinic.
