EligibilityTo find out if you or your loved one can benefit from the RMH Hospice program, review some of the requirements below and call our office at 433-4429. We’re happy to answer any questions you may have.
Some of the requirements for hospice care include:
- An illness that is no longer curable and the patient is seeking comfort measures for his or her end-of-life care.
- A physician referral. The patient's physician will manage the hospice care.
- A caregiver, usually a family member, who agrees to be involved in the patient’s care. RMH Hospice will provide training to the caregiver to help with their loved ones’ care.
- If care is going to be provided in the patient’s home, RMH Hospice will help determine if the home is adequate for safe and effective care. They can also help coordinate needed medical equipment, supplies, community services and hospital care. When needed, hospice care can also be provided in assisted living facilities and some nursing homes.
- Many people are eligible for hospice care coverage under special benefits provided to them by Medicare, Medicaid or health insurance. No person will be denied RMH Hospice services on the basis of race, color, national origin, handicap, age or ability to pay.
